Thank you for trusting and allowing Markel to service your policy and business. We know that your time is very valuable and thank you in advance for completing this online premium audit form.
To prepare you for your audit we’ve created the checklist below to help you organize and simplify the process. The form will take approximately 20 minutes to complete. Please note that once you start the form there is no option to save and complete later. You will want to make sure to set aside time to complete it at once. We ask that you submit the form including the supporting documents within fourteen (14) days of expiration of your policy. Failure to complete this audit may result in a penalty being assessed on your policy.
Checklist of documents
- Payroll Report that indicates gross payroll for each employee during the audit period. (If your policy effective dates don’t match your payroll periods, please use payroll records dating to the nearest 1st of the months. Examples of payroll reports examples are QuickBooks Payroll Summary Report; ADP / Paychex Employee Earnings Summary, etc.).
- IRS Form 941s/Quarterly Federal Payroll Tax Reports for the last four quarters.
- 1099’s, amount paid to subcontractors, contracted labor, or casual labor, etc.
- Subcontractors’ Certificates of Insurance
After submitting your audit
Your completed online audit form will be emailed to you and our auditor. Please note the auditor may contact you to verify or ask for additional information. Once processed, you will receive the Audit Explanatory Statement.
